    qtmonitor: add per-IP and per-server clear controls to Login Attempts

    Right-click individual rows to clear specific IPs. Clear dropdown
    button with per-server entries and "All" option. Publishes IP to
    host-level clear topic for per-IP, server-level clear for per-server.
